條款及細則
本人（本合約之簽署人）同意本合約之所有條款及細則，本人清楚知悉購買產品及療程或服務的內容。本人知悉已支付了的療程按金將不會得到任何退款或以任何形式兌換。本人明白本人有責任閱讀並遵守全之妍醫學美肌中心之客人守則。本人已知悉所購買之服務兌換有效期限為6個月內。本人在此保證現時是身體及心理狀況良好，並適合使用及進行任何本人自願購買的針劑、激光、彩光及其他醫美療程。
Terms and Conditions
I（the undersigned of this contract）agree to all the terms and conditions outlined herein. I acknowledge that I have received and understood all information regarding the products and treatments or services I have purchased. All deposits paid for the treatment are non-refundable and non-exchangeable. I understand that I have the responsibility to read and comply with all terms and conditions. I acknowledge that the validity period for redeeming the purchased service is within 6 months.I hereby guarantee that my physical and mental health conditions are suitable for any injections, laser treatments, intense pulsed light, or other aesthetic medical treatments that I have voluntarily purchased and participated in.
